A light bleed a few days before your period, and straight away the question: is this your period coming early, or is it already implantation? Here you will find when implantation bleeding can occur, what it looks like, how to tell it apart from your period, and when a test gives you clarity.
Data box: What studies show about implantation bleeding
A US study had 221 women trying to conceive collect a urine sample every day and keep a bleeding diary. This made it possible to determine the day of implantation hormonally. Of 151 pregnant women, 14 (9%) had bleeding in the first 8 weeks, mostly around the time of the expected period, and hardly any on the day of implantation. The researchers found no evidence that implantation itself causes bleeding (Harville 2003). 12 of the 14 women had a baby.
What is implantation bleeding?
Implantation bleeding, also called implantation spotting, refers to light bleeding when the fertilized egg implants in the lining of the uterus. The explanation behind it: during implantation, small blood vessels in the lining are damaged.
Health websites such as the British NHS, Cleveland Clinic, and familienplanung.de list it as a possible early sign of pregnancy (NHS signs of pregnancy, Cleveland Clinic 2024, familienplanung.de bleeding 2024). The gynecologists’ information site Frauenärzte im Netz only mentions light spotting at the usual time of the period, without attributing it to implantation (Frauenärzte im Netz 2018).
Nobody knows exactly how common implantation bleeding is. Cleveland Clinic writes that about one in four pregnant women has it (Cleveland Clinic 2024). However, that figure matches the share of all bleeding in the first trimester, most of which does not occur until the 6th to 7th week of pregnancy (Hasan 2010). For bleeding at the time of implantation itself, the only figure is the one from the data box above.
When does implantation bleeding occur?
The fertilized egg implants 6 to 12 days after ovulation, in 84% of pregnancies on the 8th, 9th, or 10th day (Wilcox 1999). Bleeding caused by implantation would therefore be possible at the earliest about one week after ovulation, not directly after sex or ovulation.
For a 28-day cycle with ovulation on day 14, this means:
| Days after ovulation | Cycle day | What happens |
|---|---|---|
| 6 to 7 | 20 to 21 | earliest implantation |
| 8 to 10 | 22 to 24 | implantation in most pregnancies |
| 11 to 12 | 25 to 26 | late implantation |
| 14 | 28 | day of the expected period |
The health websites give different figures: 10 to 14 days after ovulation (Cleveland Clinic 2024), 1 to 2 weeks after fertilization (ACOG 2021), or around the time your period is due (NHS signs of pregnancy). In the study from the data box, the bleeding mostly occurred around the expected period (Harville 2003).
The Pregnancy Test Calculator works out your own implantation window and the right test day.
What does implantation bleeding look like?
The descriptions on the health websites largely agree (Cleveland Clinic 2024, NHS signs of pregnancy, TK 2025):
| Feature | Implantation bleeding | More likely something else |
|---|---|---|
| Color | pink, light brown, or dark brown | bright red or dark red |
| Amount | a few drops or spotting, a panty liner is enough | pad or tampon needed, bleeding gets heavier |
| Consistency | thin, often mixed with discharge | clots |
| Pain | none or a slight pulling sensation | period-like cramps, severe pain |
| Course | consistently light, stops on its own | starts light and gets heavier |
Brown is the most commonly described color. Blood turns brown when it is somewhat older and drains slowly. Brown mucus or brown discharge therefore fits the picture of light bleeding, but it is not proof of implantation.
Pink comes from a small amount of fresh blood mixing with discharge.
Bright red and heavy does not fit implantation bleeding and points more to a period (Cleveland Clinic 2024).
You will find plenty of photos of implantation bleeding in forums. But you cannot tell anything apart from a picture: a light period, bleeding between periods, and bleeding in early pregnancy look the same.
How long does implantation bleeding last?
According to Cleveland Clinic, it lasts from a few hours to two days at most; sometimes it is just a single spot (Cleveland Clinic 2024). The German health insurance fund Techniker Krankenkasse mentions a few days (TK 2025). There is no study that measures the duration of implantation bleeding itself.
For comparison, there are figures on all bleeding in the first trimester: just over half (51.5%) lasted only one day, and another 17.2% lasted two days (Hasan 2010). A period lasts 5 days on average (Dasharathy 2012).
How heavy is implantation bleeding?
Light. In the study from the data box, the bleeding was mostly light and could hardly be mistaken for a period (Harville 2003). Among all bleeding in the first trimester, too, 75.6% was only spotting, 18.4% light bleeding, and 6.1% heavy bleeding (Hasan 2010).
According to the descriptions on health websites, there is no such thing as heavy implantation bleeding. If you need a pad or tampon, soak through a liner, or pass clots, it is more likely your period (Cleveland Clinic 2024). If you already know for sure that you are pregnant and are bleeding as heavily as during a period, get examined right away, see When to see a doctor?.
Implantation bleeding or period?
| Implantation bleeding | Period | |
|---|---|---|
| Timing | around implantation, 6 to 12 days after ovulation, often shortly before the expected period | around the expected day |
| Duration | hours to 2 days | 5 days on average |
| Flow | spotting, stays the same | heavier at first, then lighter, heaviest in the first 3 days |
| Color | pink or brown | bright red to dark red, often brown at the end |
| Clots | none | possible |
| Pain | none or mild | period cramps are common |

At the start, the two cannot be told apart with complete certainty. Many periods begin with brownish spotting. If the bleeding gets heavier and bright red on the second day, it is very likely your period. If it stays light and stops after one to two days, it can be either, including spotting before your period. A pregnancy test gives you clarity from the day your period would have been due.
A normal period during pregnancy, on the other hand, hardly ever happens. Bleeding in early pregnancy rarely resembles a period (Hasan 2010).
Other reasons for spotting
Light bleeding in the second half of the cycle often has other causes:
- Spotting before your period. If your period starts one to two days later, it was usually this kind of spotting.
- Ovulation bleeding. In one study, 4.8% of women with regular cycles had bleeding in the middle of the cycle (Dasharathy 2012). It occurs around ovulation, so about one week before any possible implantation. More on this in the section on ovulation bleeding.
- Contact bleeding. After sex or a gynecological examination, the cervix can bleed slightly, including during pregnancy (ACOG 2021, familienplanung.de bleeding 2024).
- Chemical pregnancy (biochemical pregnancy). A pregnancy that ends very early after implantation can look like a slightly late period. In a study with daily urine tests, 22% of pregnancies ended before they were clinically recognized (Wilcox 1988). More in the guide on chemical pregnancy.
- Ectopic pregnancy. About 1 to 2 in 100 pregnancies implant outside the uterus (familienplanung.de ectopic pregnancy 2025). The bleeding is often dark brown and watery, comes and goes, and can be mistaken for a period. It is often accompanied by one-sided lower abdominal pain; symptoms usually begin between week 4 and week 12 of pregnancy (NHS ectopic pregnancy 2022).
When to test after implantation bleeding?
Not on the same day. The body only starts producing the pregnancy hormone hCG after implantation, and on the first day after implantation there is usually still too little of it, even for an early test. In the following days, the level in urine rises quickly, at first roughly tripling from one day to the next (Nepomnaschy 2008).
- Early test with 10 mIU/ml: a few days after the bleeding at the earliest, ideally 2 to 3 days before the expected period.
- Reliable: from the day your period is due. Even then, a test finds at most about 90% of pregnancies, and 97% one week later, because some embryos implant late (Wilcox 2001).
- Negative, no period: repeat after a few days (NHS pregnancy test).
The pregnancy test comparison shows which tests detect a pregnancy early and what they cost. The Pregnancy Test Calculator works out the test day for your cycle.
Bleeding after embryo transfer
After IVF or ICSI, some women bleed before the test day. In a study of 1,858 singleton pregnancies after transfer, 17% had bleeding before week 8 of pregnancy. If the ultrasound was normal, this did not increase the risk of miscarriage (Pollie 2023).
What this means for you:
- Stick to the test day. Your fertility practice decides when to test. The London NHS clinic Guy's and St Thomas', for example, has patients test 16 days after egg retrieval, even if bleeding starts before then (Guy's and St Thomas' NHS).
- Keep taking progesterone until the practice tells you otherwise. The European professional society ESHRE recommends giving it at least until the day of the pregnancy test (ESHRE 2025).
- Do not test too early. A trigger shot with hCG can be detectable for up to 8 to 10 days and cause a false positive test (Guy's and St Thomas' NHS).
- Contact your practice if the bleeding becomes heavy or you get pain. After IVF, the share of ectopic pregnancies is about 1 to 3% (Guy's and St Thomas' NHS).
The section on testing after the transfer in the Due Date Calculator shows which days apply after a transfer.
When to see a doctor?
If you already know you are pregnant, have any bleeding checked by a doctor, advises familienplanung.de (familienplanung.de bleeding 2024). Spotting on its own is usually harmless: in a large study, light bleeding and spotting in the first trimester were not associated with more miscarriages, but heavy bleeding with pain was (Hasan 2009).
Go to a hospital right away or call emergency services if you have (familienplanung.de bleeding 2024, NHS bleeding in pregnancy 2026, NHS ectopic pregnancy 2022):
- bleeding as heavy as a period or heavier
- severe or one-sided abdominal pain
- pain in the tip of your shoulder
- dizziness, weakness, fainting, or circulatory problems
These signs can point to an ectopic pregnancy or a miscarriage.
What is true and what is not?
| Claim | What the data say |
|---|---|
| Every pregnant woman has implantation bleeding. | No. In the study with a measured day of implantation, only 9% had any bleeding at all in the first 8 weeks (Harville 2003). |
| Implantation bleeding proves a pregnancy. | No. Spotting before the period and in the middle of the cycle also happens without a pregnancy (Dasharathy 2012). |
| Implantation bleeding comes shortly after sex. | No. Implantation begins 6 days after ovulation at the earliest (Wilcox 1999). |
| Bleeding in early pregnancy means a miscarriage. | Usually not. Spotting does not increase the risk, but heavy bleeding with pain does (Hasan 2009). |
| No bleeding means no miscarriage. | False. About half of women who miscarry have no bleeding beforehand (ACOG 2021). |
| Bleeding after embryo transfer means it did not work. | Not necessarily. Still test on the specified day (Guy's and St Thomas' NHS, Pollie 2023). |
Frequently asked questions
When does implantation bleeding occur?
If at all, around the time of implantation, that is, 6 to 12 days after ovulation, usually 8 to 10 days after (Wilcox 1999). In a 28-day cycle, that is roughly cycle day 20 to 26, often a few days before the expected period.
What does implantation bleeding look like?
Pink or brown, very light, usually just a few drops or spotting without clots. A panty liner is enough (Cleveland Clinic 2024). Bright red, heavy bleeding points more to a period.
How long does implantation bleeding last?
A few hours to two days at most (Cleveland Clinic 2024). If the bleeding lasts longer and gets heavier, it is probably your period.
Is brown spotting implantation bleeding?
It can be, but that is not certain. Brown is older blood. Brown spotting can just as well occur before a period. A test from the day of the expected period shows whether you are pregnant.
Can implantation bleeding be heavy?
Not according to the descriptions on health websites. Heavy bleeding with clots points to a period (Cleveland Clinic 2024). If you are already pregnant and bleeding heavily, get examined right away.
When can I test after implantation bleeding?
An early test can turn positive a few days afterwards at the earliest; a test is reliable from the day of the expected period (Wilcox 2001). The Pregnancy Test Calculator works out the test day.
Does every pregnant woman have implantation bleeding?
No. In the only study with a measured day of implantation, only one in eleven pregnant women bled in the first 8 weeks, and hardly any on the day of implantation (Harville 2003).
Can I get my period despite implantation bleeding?
If your period starts after spotting, it was usually not implantation bleeding but spotting before your period. A very early miscarriage is also possible: a chemical pregnancy, which looks like a slightly late period.
